How Do We Get Care?

Enquiry Process

Having to choose care or support services for the first time or even changing your provider can be a very worrying time. So we have made a step by step process for you and/or your family/advocate or friend to follow:-

  1. You or a family member can enquire to use our services, or a social worker (Essex Council Council) , Community Nurse, GP or other professional can enquire on your behalf - If you have any difficulties with this please call 01255 424245 or 01255 476107 where there will be someone here at Montécare to help you with your enquiries.
  2. Once we have recieved your enquiry we will arrange for a member of our assessment team to meet you at a time and place of your choosing to find out more about your requirements and answer any questions you have may have be they major or minor.
  3. We will collect all of the information we need with the support of yourself or whoever else that you would like to be a part of your care package. We will write up a Care Plan for you and then we will also write up two Risk Assessments. One for yourself and how you can manage and what you can and can't do and need support with and the second Risk Assessment will be about the Environment that you live in. This is to ensure everybody is safe and looked after (Yourself and the Carer).
  4. Once you feel you are completely happy with your Plan and we have all of the information needed, we will then discuss with you about the times you would like to have your support and days. Also, we will discuss charges and your Contract.
  5. Your Care Plan can be changed as and when required because you may feel you need more or less support and we will then come out again and discuss this with you.
